I always got the sense that Death wasn't too fond of Falling, which is why she kept things formal.
She insisted that Asa and Yoru and Yoshida call her "-chan" also. Her friends at the school did that, too.
It's not really that exceptional on her part. It's actually Falling that's the exception.
