100% sure on this one.
"=== SYSTEM PROMPT ===" contains the full set of instructions and context given to the AI.
The format with "=== Dialogue History ===" is something I made myself to improve readability in the logs.

Tested on OR.
I put what I had in the System Prompt into a tokenizer:

Here are the prompts sent:
OR counted 18640, but I think it's safe to say they're identical.
The first two (18640) were during scene generation—the second one is a retry. No duplication.
19347 was during Q/A, right after the first line of dialogue. I tried two regenerates—no duplication there either.

So either:
1) Your script is messed up, or the connection between it and the VN is.
2) Something happened in your save that caused the prompt to be sent twice.

To check, you could either:
A) Send me your save so I can test it on my end.
B) Try a new game and see if the same thing happens.
