Anime character design is an imitation of Disney cartoons from the first half of the 20th century either directly or indirectly through prior anime.

AOT's plot point of ethnicity and differentiating Mikasa from the rest of Shiganshina as an anomaly, or the weird stereotypically American character that is more American than regular Americans)? Just giving western settings, aesthetics, name, or lifestyle doesn't seem enough.
If it's an anime set on Earth I just assume they are either Japanese, unless there is something like character name, architecture, food or religion that is more commonly associated with another place .
