This is pretty typical for dementia. Basically, at the end, you're shut up like a clam in it's shell. I've worked with people who deal with extreme dementia for a while now and it's possible for him to continue living for a number of years even in that state- though he will need frequent assistance to keep from developing sores. He almost certainly looks around while he's being fed, may smile or (much less frequently) express sadness. At this phase, negative expressions are very uncommon in my experience. Instead, every moment he is awake will likely one of constant wonder: Every flower seen and smelled the first flower. Ever smile and touch the first touch. He has a beautiful family who loves him. He is a traveler who does not move, surrounded by beautiful things and loving people who will come and visit him until the last day when a voice will call him by his name and he will look up and smile in recognition.
