There are ways to add new bones, as shown by other mods like IVCS and certain cape mods that have extras as well. They come in the form of these j_ex bones, the Veil of Eternal Devotion has them for example.
When I was digging into the idea for myself, it wasn't all that well documented but someone was showing how to add bones through the vfxeditor(?) plugin. To answer your question, it's not all that simple and pretty involved with vfx editing and custom rigging in blender. I can't for the life of me remember where I saw that resource though so I cant be of more help.
