A - Clickbait, title or thumbnail
B - A low click rate
C - Short videos that aren't Shorts
D - That the majority of your views doesn't come from YouTube itself (bots)

Two or more items combined reduce the impressions generated with each of your videos, but it takes time for a channel to truly go into crisis.
