Enemy health should be consistent across difficulties, I don't think it's as extreme as what Half-Life or Blood does. Harder settings definitely add more enemies, makes them more aggressive, and more reactive (quicker to shoot+dodge). I'd have to test if it swaps spawns depending on difficulty.
