The effects of PTSD aren't usually fully realized until after the threat is gone/war is over and the person tries to adjust to normal life. This is even explained in the show. Kid Steven always had a new threat to deal with. No time to think about how unsafe he's grown adjusted to feeling when he's still always unsafe. It takes a while to adjust to the wider and uglier side of reality that you've gained the intelligence to more fully comprehend. The sweet ignorant joy of innocence doesn't last forever. It's only ruined if you consider growing up ruination.
