No, I did see that, and I'm genuinely unsure whether Outis' secret technique involves taking whatever sharp object near her (and not her sword?) and she happened to use a prosthetic, or the art and possibly also the writing omitted the bit about the head. Thought we saw just the front of his head but we see mostly the back as well. However

It seems that the art isn't completely accurate to the writing either. If in her canto, needlessly killing poor Effie is one of her faults that come up with zero objections on her part, then okay, but I think it's currently ambiguous. I think part of loving a character is wanting to be charitable but another part is accepting their faults also, which I'm bready to do.
