Spent 13.5 years in USAF in 1970's and 80's. Didn't fly, but handled investigation update reports to higher headquarters on a regular basis along with reports of other problems. Typical investigation conclusions were one of three results. Pilot error, proven mechanical failure due to material issues, mechanical failure due to errors by maintenance personnel. God help anyone tagged with result 1 or 3 because they and their career were instant toast. The same for any pilot deemed to have ejected without proper steps to avoid civilian damages or potential damages. That was not just for fighter jocks, that was all pilots with a "Mishap Aircraft". Maybe it was different for Navy or Marine pilots, didn't deal with those cases. And maybe it has all changed since I left in 1985, but I sincerely doubt that.
