People IRL can't help being ugly if they are born deformed or suffer traumatic injuries. It's not their choice.

People in videogames have no reason to be ugly, making a character ugly in a videogame is always a conscious decision on part of the developers. Why is it done? Well, sometimes it is to tell a story, or maybe the game just has that kind of a design where grotesque or deformed characters are necessary to communicate a theme or just gross the player out, or maybe for fun.

Making characters ugly out of spite is not a legitimate design decision and it is rightful to call it out.
