The Iranians already have large underground bases, mountain retreats and non-negligible air defenses.

The nuclear bunkers are up to 90 meters underground (300ft in burger units). Besides, the nuclear program is not the only concern Israel has here, Iran is their enemy in general. Regime change requires friendly intelligence access to the country, so that is not going to happen.
