The injuries and diseases mod just kinda randomly gives everyone canceraids over a period of time, not really very sophisticated under the hood
If events were less retarded i would make diseases have a diceroll chance of happening after certain ingame events
>lord/character entered towns or villages of another culture
>lord/character travelled a long distance in a short period of time
>lord/character was outside while harsh weather was taking place for over a three days in a row
>lord was in captivity or injured in the field for several weeks in a row
and parties and caravans could do the same for settlements, it's just too much focus on the diseases themselves as debuffs and not enough on the vectors of transmission which could be far more game changing, like pandemics could lead to fatally crippling the world economy and fuck up any faction that's doing TOO well.
