I intentionally structure my setting to have a mostly-classic-fantasy core with increasing weirdness the further away you go from the core. It's very much an artificial thing - the reigning pantheon in the core is bitchy, arbitrary, as they're Gods, and they have a specific aesthetic sense they enforce on the world where they can, because aesthetics affect them on a fundamental level.
The first giveaway that there's a bit more to the world than elves, dwarves, occasional dragons, gods and the undead is that the orcs are plant people with hedges for heads if they're from a city.
