She instinctively knew how to block scarlet or how to cut snow's white attack. Yet she was shocked or surprised when Nayuta attacked, someone who Lilith has never seen before.
As for why she ran away from Dorothy, I don't think it was because she was bleeding. It's because, unlike the rest of Pioneer, Dorothy accepted that Lilith was dead and she would kill her if she has the chance.
