Worse. I would argue that in America's "acute collapse" we would see things far worse than a 3rd world country. See, in a 3rd world shithole, they have a system that works, a culture that is adapted to their shitiness. An immune system that can handle fecal-flavored water and rotten meat for food. I would argue that thing would be worse for many people, especially urban scum, for a while until things got better. How long that might be and how severe is anyone's guess. We have zero low-tech infrastructure, no cultural adaptation to handle this, so keep that in mind.
