Theoretically, yes.

Practically its not that obvious it mostly depends flaw in specific image viewer and abusing it by embedding into image some sort of malicious command that downloads malware or is malware itself.

So technically in order to make someone infected by just clicking and image, that person must have specific version of specific image viewer and send im image with embedded keylogger specifically written to run by using that specific flaw and that means to many variables to make that type of attack easy to do outside laboratory environment.

Tl:DR - You can get infected by clicking on image but chances are so low its practically impossible.
