The problem was the Vapaus worked perfectly. Lilith's NIMPH was the only thing preventing the Queen from assimilating her head, and then Nayuta told Nihilister to feed the Queen the rest of Lilith's body with the Vapaus. That just removed Lilith's NIMPH and then nothing was preventing the Queen from possessing it. Weirdly, Nayuta is the one who brings up the fact Lilith's NIMPH is still fighting the Queen, which is the reason why the Queen hasn't replaced her original head with Lilith's head yet. So Nayuta already had all the information she needed to realize that her Tainted Grail plan was a colossal mistake that would literally hand the Queen the most powerful Nikke to be her new body. There's no excuse for a blunder of this magnitude except that Nayuta knew this would happen and planned for it, but she actually acts surprised afterwards. And unfortunately, Nihilister was caught in the crossfire of Nayuta's blunder. It's a complete waste of Nihilister's great moment of eating Indivilia to get her revenge finally and breaking free of the Queen's control.
