You should also be using hentai enchants and both rare Inner Fire enchants (Glyph of Inner Fire, Improved Inner Fire)
Shadowfiend+Grasp of Darkness really hurts
Make an WeakAura for Fear Ward to remind you to refresh it every 3 minutes. Would help too if you made one for your dots
