If I take this question at face value, I have to interpret it to mean "can a person alter their *medically definable manifestation of innate sex* via surgeries" and presumably, hormone treatments.

If that is the question, then I think that we could agree that it is not presently possible, or logically foreseeable, that we can alter a persons chromosomal profile at the cellular level to reflect a particular sex other than the one they were conceived as. This process is selected during germ cell synapsis (or de-selected, more accurately), and what follows after conception is the physical manifestation of anatomy as a consequence.

However if I interpret the question as "can you alter a persons *gender identity* with surgery" etc, then I am confronted with the trans paradox.

That is, how do you alter something into itself? It becomes a problem of maintaining a frame of reference. If they are female despite being anatomically male, then their gender is not altered; they remain a female. Yet a process is occuring that seems simply cosmetic. So if they are being altered, then how is their gender changing? For such a person to claim they have altered their gender implies a paradox, that they were previously in a state that cannot be altered before they altered it. If you claim they altered the appearance, then the gender never changed, only the appearance.

Herein is the paradox. It becomes a problem of maintaining a frame of reference. And so I don't know how to answer it, or if it can be answered. Sometimes the syntax of the question, as pedantic as this sounds, can create this effect ("What happens when an unstoppable object meets an immovable post?")
