Because it's a controversial issue in the States. Some support it, some don't. That's why it varies between states and isn't in a federal thing. Some would argue it's unconstitutional, some would say it should apart of a decent civilized society. It's just not that simple. I may personally agree on a federal subsidized system of healthcare (that isn't Canada's or the UK's) but tens of millions may not. That's just the reality of it.
