Jesus here is not telling anyone to gouge their eye. He says “IF”. And only if. But the eye does not cause someone to stumble. Neither does the hand. A man blinded by cataracts will still lust. And a man with no hands will still steal. The hand and the eye listen to the heart. And this is what is causing a man to stumble

>19 For out of the heart come evil thoughts, murders, acts of adultery, other immoral sexual acts, thefts, false testimonies, and slanderous statements.

So you see the heart is the actual problem. It causes us to stumble. But removing the physical heart is barbarian and will not make you holy or free you from your sins. You need a spiritual surgery. Your spiritual heart must be changed. “Cirumcision of the heart” but if you cant even do surgery on your own physical heart, how will you do it to the spiritual?
