A big part of design is suggestion.
In a time when games didn't have the budgets of government projects and weren't obsessed with realism, you're not gonna be able to have a character that actually looks like person. So the idea is to suggest things that aren't really there and have the players mind fill the gaps.
Damsel is a perfect example of that. Her expressions, voice, mannerism they all hint at what isn't there but is copacetic among players.
