I think that's what crpg worlds tend to be, tho. Like, in DAO for example, in the elf origin an important noble's son comes to the elven part of the city the day of your main character's arranged marriage is happening, and kidnaps your fiancé, the fiancé of your cousin and the sister of your cousin. Then, the first thing you have to do is go to the noble's manor to save them. But, while you get in time for both MC and the cousin fiancés, the female cousin has already been gangbanged by the noble's son and his friends.
Arcanum has a lot of fucked up shit in them too. Like there's a quest where you have to unmask a conspiracy . There's a hidden island that was used for systematic breeding of half-ogres, ruled by gnomes who are rich bankers and businessman who have great power in politics in that world (does that reminds you of someone?). Women were kidnapped and taken to that island to get forcefully breed by pure ogres in order to create a new race (the already mentioned half-ogres). And also experimented on. Most of them died at childbirth because their bodies were not ready for the size of the "baby". You discover all that shit through notes and other evidence on the island. When you come back to the mainland to communicate with the guy that was helping you to reveal everything to the public, he was not to be found anywhere. He vanished. Instead, in his house, you find a gnome there who tells you to drop the case because you're a nobody and the public would never believe such "bullshit" coming from your part anyway. and that's how the quest ends
