can anyone help me fill the hunter's journal? I'm pretty sure I did all major bosses, just missing 4 random entries to finish it

also I'm missing 2 relics and 3 mementos if anyone knows anything about those as well, I tried looking it up and it seems like there's a sprintmaster and a guardian one, I've already done all 3 races so I don't know why I didn't get it still, as for the guardian I found him once in greymoor so maybe there's a quest tied to him after you beat the boss
