I feel like a high fantasy DnD setting would have access to better ways to transition that didn't leave scars. I mean, couldn't you just get someone make you a girdle of gender change(are those even still a thing?) or polymorph you? It's the same weirdness behind the wheelchairs.

I could see it working if you had a character in a relationship with an NPC that started before the game began and/or they were distant. EG a character sending money to a crippled wife at home or on a quest to rescue his fiancee or whatever. But RPing that intense beginning stage of a relationship with the GM while everyone else sits around at the table feels very cringe on multiple levels.
Then again, I feel like this book is aimed at people who like to pretend to be tieflings over Discord and not actual serious gamers exploring dungeons.
