Why is this bad? Older RPG frequently punished players for making retarded or non-optimal choices and they were better for it, some even view it as the golden years of video games. It gives choices in your game more weight when there are tradeoffs than everything being equal like modern day RPGs.

I disagree, you saving the orphanage is a great reward. There's a small quest in Witcher 3 that rewards you with this picture, it doesn't impact your gameplay in any way but its something that I fondly remember from my first playthrough from almost 10 years ago. Being virtuous is its own reward.
