It’s interesting to hear your thoughts. Of course there’s no right or wrong answer. NTR for me is at its core a guy getting cucked in a manner that arouses extreme, complex feelings in the reader.

Picture the Middle Ages. A young, fun, intelligent peasant woman gets discovered and married to an old, boring asshole Baron. She cheats on him with a young, fun, intelligent peasant guy. That isn’t NTR to me. In that story the Baron would be the bad guy, and the young couple star-crossed lovers.

NTR would be the young woman getting married to the young guy, and the Baron sees her before the wedding and exercises his right to prima nocte. OR, if that’s too close to a rape scenario, the Baron is selling off land and the peasant family will be homeless before winter, so the girl goes and offers herself - WILLINGLY, but RELUCTANTLY, and doing it for love of family rather than lust.

In this case she wouldn’t be attracted to the Baron at any point, the essential thing would be she’s defiled, and her marriage isn’t ‘special’ or ‘pure’ ever again as a result of her actions because this unspoken thing is always between her family. And even the Baron isn’t really interested any more, he’s cracked her, had her every way, he’s onto the next peasant.
