Few instances of what has been dubbed "Atlas shrugged", where a level transition kind of "freezes" into a black screen after loading, but you can hit pause and sort of see through the pause screen, so hte game isn't frozen, but the fade is stuck.
Named "Atlas shrugged" because of a boss Atlas that had this bug happen often after defeating it and going through a door behind him.
Luckily this bug is easily bypassed by reloading a checkpoint from pause menu, as it loads the new area.
And this Atlas boss is like waaaaay deep into the game and the bug has been dealt with from what I hear from developers.
