Unless something's changed recently, it happened, but also it's also not relevant. Phase 3 was a failure, so they hurriedly rushed to write everything from it away. Cyborg Noodle? Killed offscreen by regular Noodle. Her head was turned into a vase or something and stored out of sight, out of mind. That's how they treat stuff from Phase 3. It might as well be non-canon because it doesn't matter.
