'Adoptive Relationships', I combine it with 'We Are United' so you can ask random orphans to join. However it does require some manual steps with character editor. Supposedly Romance on the Rim also has an adoption feature but I've never seen it occur. Also Ender and Jakey can have kids just fine, it's just that when they got married a beggar group showed up shortly after so I thought it was a nice story moment.
