I can't say I've ever seen it happen in this specific niche case (indie eastern artist pressured by mass western attention), but that's because I'm not really knowledgeable about that.
I know a lot of big eastern games (especially the very old ones), have different releases and censorship. A more recent case is FE Engange where they censored some romance scripts for the western release.
Aside from that, there's cancel culture. I don't think I need to explain that. There's some real lunatics out there that perceive something they don't agree with as a personal slight and go out of their way to doxx you and shit. Exhibit A is gacha players.
It's basically a given that IF Black Souls does get meaningfully popular, it will attract the wrong kind of attention. The things to hope for are:
A. That it doesn't get popular in the first place
B. That the language barrier helps
C. That EMMM can ignore terminally online culture warriors
The outlook is grim.
