That's it could happen but it isn't rare for one or two clubs to concentrate all the following, money and prestige in a city. Other than London and Istanbul, no European cities come to my mind where you have more than two big clubs (which is the case for Lisbon). You'd have to look for that in Latin America, where the major cities are disproportionately more populous and wealthier than the rest of the country to absurd degrees.

Most fans outside of the big three watch football as an hobby, they know they're not going to win the championship, it's more of an weekend event where you drink beers and eat a bifana with the lads.
They have their rivalries and in the end are fighting for Europa league spots or avoid relegation.
In a way I think they suffer less than the big three supporters that only want to win no matter what, but two of them will always lose.

Delta Cafés played a major role here, until they figured out it made no sense to keep financing Campomaiorense.
Without industrial support, you definitely can't climb the football pyramid in Portugal. That's why we see so many random teams from the north and upper centre of the country. Lots of industry there.
In Alentejo there's not a lot of industrial centres rivaling the northern ones. Maybe only Sines and Évora.
