I just prefer historically accurate medieval level aesthetics, culture and societal structure for even high fantasy settings because that’s what I find more appealing. I don’t care for traditional high fantasy aesthetics very much.

I don’t care, and it should apply with urban fantasy too by that logic, they made it work with Arthurian Romances and Charlemagne songs you can make it work now. And you can mix it in with an extremely exaggerated fantasy-version of real medieval occult beliefs to make it more interesting

I get what you want to convey, but I can't get over the fact they make the underwear look like diapers.
I'd rather have my character wear a pre-medieval tunic which would today attract sneer for its likeness to a woman's skirt.

It depends on the fit. The white part is usually concealed by a longer shirt/tunic/skirt, and in time fashion favored shorter tops which led to the exact issue of people seeing the unfashionable baggy underwear, and there was a solution to that tool
