So what is REAL difficulty in games? Should every game trying to be a challenge/hard have random modifiers for everything (ie number of "X atks" to stun an enemy)? Should the CPU constantly cheat against the player (as seen with some strategy/horror games)? Maybe simply overcharging the base game, such as increased enemy damage, increased enemy health, less time to finish a level, less resources, etc, is enough for a player to prove they've mastered the game mechanics (although many devs will pick this option and not test it, leading to aspects that seem broken!)?

Difficulty is encapsulated in how much effort is required in reliably performing and repeating a task, and in the chances of its success. Never quite understood the concept of artificial difficulty personally. There can be rng to varying degrees. A game might say, be harder for someone with slow reflexes than someone with fast ones. It might be harder to play properly when one is on the phone, or talking to someone, and sure, that doesn't belong to the game and is added in, but ultimately every player and context is different, so eh. Difficulty is still just difficulty, personal and contextual as it may be.

Attention span deficit for example can make it harder to read even medium length 4chan posts. Many such cases.

Idk, but I'm just heavily against custom sliders. It's anti-social behavior and removes from the shared experience of the game. Imagine if other art forms, such as movies, books, paintings, music had them? Could you even proclaim you experienced the same piece?

Games aren't art. You wouldn't call Monopoly art. And I'll bet you've played with a younger cousin with handicap rules before.
