Depending on the game and its modding community, the mods can range from simple graphics alterations (think titty mods for Skyrim, or ReShade makeovers for any game really), to these full-blown brand new games (TCs).
Half-Life 1-2 mods are best known for the latter type, with many of them later turning into brand new standalone IPs (ie. Counter-Strike, Team Fortress...).
The in-between variant could be these gameplay re-balance and expansion modifications, something you'll find a plenty for, say, the STALKER trilogy.
The undeniable king of modding still must be the original 1993 DOOM. People still mod and customize the game and its engine to this day, even release brand new games using its forked source code.
One more thing worth mentioning:
mods and source ports of old CONSOLE games, such as Mario 64, Zelda 64, Metal Gear Solids...
We even got online functinality revival projects for games like Resident Evil: Outbreak, Metal Gear Online 1-2, and who knows what else; all of them have been "dead" for two decades now!
