Sure it could have. He just wrote autistically clean code that wasn't actually needed. The Duke 3D engine was 'held together by bubble gum and duct tape' and it was better than Doom in every way.

Wolfenstein modders. They put it in Carmack's head that players wanted to mod their games and that supporting modding could extend a game's life and popularity, so when they made Doom, id Software came up with the WAD system to make it easier to mod, and when the fanmade Doom Editor Utility and WAD editors popped up shortly after Doom's release, id Software was tolerant of those efforts, and thus was birthed the first real modding scene in gaming history.

People forget Romero codes as well, he wasn't just the level designer. Carmack was obviously better at it but Doom without Romero wouldn't exist. Carmack just as easily could've ended up in some other random company making RTSs or simply never entered the industry in the first place and he'd be just as happy.

He never really recovered from Daikatana. Between the game sucking after there being a lot of hype and getting the reputation of being up his own ass because of that one ad, he basically left the industry. Some of the newer games he was involved in are more from his family, he's just lending the name.

Because it was good to have and it was eventually made open source. But that doesn't mean there weren't other programmers who couldve produced similar results (keep in mind that Ultima Underworld for example was out before Wolfenstein 3D). Perhaps the game would've been less performant, which would be bad and limit its popularity, but the game itself would be no less enjoyable once hardware advanced enough to compensate. But the game would never have the same appeal if it had different designers.

Rage, which has amazing art style was rushed out as a buggy bugthedsa piece of shit for consoles.
Probably done entirely on purpose because they didnt want Rage competing with Fartout
