DM2 is hard to say because while you're on a "cloudy" path there's no real ground to make what you're in a sky
also an enemy respawned and interrupted the fireball trap powering the crystal so now you're teleported out :^)

Catastrophic breakdowns in large facilities, especially when emergency lights (bonus points for harsh reds) are one of your few remaining protections against darkness that would otherwise swallow you up. Stuff like the beginning of Doom 3 or the end of F.E.A.R.
Or maybe I'm more just a sucker for top secret research facilities, a mix of sterile science labs and gritty industry/maintenance
