NPCs that go around the world doing what they need to survive, doing their jobs, etc.
They harvest resources and bring them to towns, merchants bring them from towns to cities and other towns etc.
NPCs at the top of the hierarchy command the other NPCs to do what they prefer, they have armies and fight other elite NPCs for territory and resources.
They expand colonies into the wilderness and fight animals/monsters.

I mean this is the most obvious formula to make a real fucking open world game, but it seems like no one has even tried.
Who the fuck cares if its a broken mess, it would be fun, groundbreaking, and eventually get fixed.
HOW THE FUCK has video game culture and industry stagnated to the point we very clearly have the technology to make this game work but NO ONE IS FUCKING TRYING TO MAKE IT

The closest I've seen to this is a game called Space Rangers 2, which is a multi-genre space adventure game. It has a huge space map and the main story is that there's a race of robots that are trying to destroy all orgsnic life and you can monitor their advance just by looking at the map. The main story can progress without you - on easier difficulties you can "win" without ever participating in the war against them, see them pushed back from every inhabited system and read in the news about npc pilots taking down robot faction bosses. Alternatively, on higher difficulties if you ignore them you will eventually see every system fall to them and you lose the game.
The economy is also real in this game - not super autism-deep like in EVE or something, but it at least feels alive. Each inhabited planet in each system wants some sort of resource, and the demands can change over time. You can go to trade centers and purchase market analysis for systems, which will show you trends so you can see what you can buy for cheap, and where to transport it for profit - that is if you wanna play as a trader. If you wanna be a pirate you can use the same analysis to figure out potential trade routes since npcs will also try to fulfill planets' needs.
Basically what I'm saying is that the world lives on its own without your input and you can use it to your advantage.
