Unless you were an educated man who knew the details of the royal family tree, the limit of your exposure to the royal family would be knowing royal written decrees, seeing a picture of them if an illustrator put his likeness to paper or stone in a place you could access, and maybe seeing the royals during a parade or public address, if you were in a place of such note that the Fire Lord and his family decided to parade through or address within that locale.
We know from the series that Ozai had his likeness in schools as a matter of nationalistic pride, and of course people would know *of* the royal family, but how else would the average citizen know the face of members of the royal family? It's not like you can just check their instagram page my dude.
