It's improved substantially over ds1. Things are smoother and more fleshed out. Menus are easier to navigate, actions take less time, way fewer cutscenes. Most exposition dumps are put into the text files so you can read them later or ignore them. Combat is substantially improved and more viable but sneaking/going around is also viable. You can skip all the cutscenes. If you liked DS1 gameplay you'll like 2.
The plot is pointless and the acting is worse, but you can skip most of it to just play the game.

The world is more dynamic (day/night cycle, weather effects the world by making rivers bigger, dust storms, etc). You can connect ladders to get further. You can take your entire backpack off and stash it to focus on taking out bandits (new mules) or getting up a difficult cliff to throw a rope down and get your cargo to keep going. Most of the PCC buildings have added features, like remote viewing on watchtowers or BT repelling shelters. You get substantially more equipment to start off and quickly unlock a lot more, including more weapons for humans and BTs, very early on (like, the first hour). You level up your skills more simply now and get much clearer feedback about what improves them and what they effect.

A lot of people complained about the cutscenes in DS1 because there were so many and the got in the way of playing the game. They were also very annoying to skip. I enjoyed them and am enjoying DS2, but it's a distinct improvement in 2 that you have the freedom to just skip the scenes, read a short summary of what's going on, and keep playing if you like the gameplay but not Kojima's movie directing skills.

It's a spirit medium stuck in a doll, so it's mostly Kojima's way to add a third person to the narrative considering Sam's mental state and repatriate powers, along with navigating them. It feels like a useless addition mostly due to the first game working just fine without it. In larger sense, i don't particularly like the cast getting bloated so much as the world design is once again heavily carrying the rest of the setting on it's back so the wacky and sometimes jarring characters really don't do the narrative any favours. It's funny, because both the first and second game are tonally all over the place. They want to be post-apocalyptic and moody, but mostly just skirt a fine line of never really establishing a solid tone, Death Stranding 2 having the amount of camp does it no favours either. Just pick a tone Kojima, goddamn...
