Gene therapy =/= genetic engineering
They had Big Boss' genes inserted as adults, it would take months for them to start showing any signs of improvement and years to truly become superior soldiers. Years they weren't going to get because the process was faulty and it was killing them.

In MGS2 you can read a book called In the Darkness of Shadow Moses: The Unofficial Truth which is written in universe by Nastasha Romanenko and the book implies that the real reason thy Genome soldiers were imprisoned was so they could be further experimented on by The Patriots.
So yeah Johnny Sasaki was one lucky fucker.

Why does everyone always forget all of the Genome soldiers were under Psycho Mantis control? Liquid even brings it up at the end of the game saying their brainwashing is wearing off after Mantis's death. So none of them are guilty or anything. They should in theory have all been let away with it as they were under mind control.
