I'm thinking he did it to himself first, giving him the face of a starfish or jellyfish or something distinctive like that. He might have done it on purpose out of some attempt at immortality, since there are a number of sea creatures that can theoretically live for hundreds of years or even indefinitely given ideal conditions.
Subsequently his creations have started worshiping him as a god-like entity. So whenever you raid the camps, caves, sunken ships, etc. where they congregate you keep finding all these idols and primitive paintings depicting some sort of vast monstrosity, only for it to turn out to be just a scientist with delusions of grandeur.
I like the idea of the scientist being a starfish-hybrid in particular because then he could lop off body parts and grow identical copies of himself as "lab assistants". It would also call into question whether or not you killed the original, or if the original died long ago, or if it even matters since there are still identical clones left anyways.
