I suspect in some cases it is incompetence rather than cowardice.
Many game devs are simply incapable of implementing "person but smaller" hence why there aren't many games that even have children in them. Given that programmers are prone to using jury-rigged, barely functional hacks to get things to work, and video game programmers doubly so, it wouldn't surprise me if the simple act of creating a smaller npc breaks some piece of game logic.

In european version of fallout 2 they turned children invisible instead of removing them. There is one town where there are little niggers standing near the door to a trader and they can pickpocket you and take ANY item.
