A lot of chapters lost their history and can't confirm who their primarch was or which is legion is their parent legion. Some chapters were even formed from the loyalist remain of the traitor legions so they want to keep the origin a secret

No, basically there are periods where the High Lord give permission to make new Space Marine chapter and they used the gene seeds they have to make the first batch of Space marine for that chapter. Afterward new gene seeds can be make in-house by harvesting from matured Space Marine. Those stubs on their bodies contain the gene seeds, one is harvested when they old enough while the other is harvested once the SM died.

I know there's a lot of speculation of chapters like the Silver Skulls (Iron Warriors), Minotaurs (Iron Warriors), Carcharadons (Night Lords), Blood Ravens (T Sons/Word Bearers), etc. But are there any confirmed cases?
