There's one step I find really weird and makes no sense. As far as I know, there's no hint towards it.

Flying off on the dinosaur is the bad end. You pussied out as soon as you got the option to, the doctor gets the flowers and unleashes a rampage of murder rabbits on the surface.
The "normal" end is just beating the game normally, you and your friends manage to escape but almost everyone still dies when the island crashes.
The true ending is the correct way to play the game. You save everyone in the end and you get a couple of really hard dungeons (Final Tunnel and Hell) to finish off the game. It does require saving Booster by skipping him when he first falls down the hole. You'll gauge how to jump the gap by looking at the red line on the floor. You also have to save Curly by getting the Tow Rope during the Core fight, attaching it to her after the fight, then draining the water from her in the Waterway rest house. You also have to fight a secret mushroom boss to restore her memory later on.

The two hints for booster are in the Pre-Fab house at the very end of the game saying "if only I survive...", which admittedly does nothing because at that point you're at the end of the game. But it's intended for repeat playthroughs.
The second hint is the red line which I picked up on immediately my first playthrough, it's pretty blatantly placed right at a jump which you can juuuuust barely make.

Actually, I find Last Cave to be a real kick in the dick. It's very well-designed but even so it asks a lot of you within a very short stretch of game. You need to execute very tight movement with the Booster v2.0 when the game hasn't really required any v2.0 movement (except maybe a little bit on Balcony?).
Actually I think not even Sanctuary tests your movement to that degree except maybe in the very first room but even then that's just asking you to conserve boost. The rest of it is just throwing a ton of enemies (or blocks) at you without much strict movement. If anything, it's the final boss rush that prepares you for that.

Well, for me it was because I've heard about what can happen to Curly and took precautions to make sure it wouldn't happen with my first run but after losing her and dropping back into an Empty Mimiga village, I wasn't so sure that I did everything right. Going a little further and hearing Moonsong, it gave me a sense of melancholy determination to keep going for her. That's just me though, everybody seems to have a different experience with the Outer Wall and that's what makes it great.
