The Roundtable Hold should have had more NPCs coming in and out of it all the time.
There are so many Tarnished running around the world that you only encounter if you take on questlines like the Manor Assassinations or Fia's trial, that you can get an entirely different expectation for how populated the world is depending on play style.
All those guys like Istvan, Patches, Lionel, Varre, etc., should have been popping in and out of the Roundtable all the time, even if they just gave you flavor text.

The Hornsent were this weird semi-animalistic race of like, nature/evolution purists, who worshipped the setting equivalent of the primordial sludge, and were trying to artificially create ascended beings by quite literally filling big jars with a sludge of victims and viscera, and hoping eventually some ascended being would emerge from one of the gore-sludge jars.

The setting god-queen hailed from a tribe of victims of the Hornsent, and when she achieved godhood, she turned it around on the Hornsent in genocidal revenge. In their extinction throes, they cursed the god-queen and her kind, to occasionally birth "omens" who are like deformed ogre-ish versions of the Hornsent.

At least I think that's how it went. In any case the Hornsent were purely just cruel and vicious cunts, one could say evil but they were more like religious fanatics of a strange faith with grotesque practices, rather than edgy evil mustache twirlers.

It honestly bothers me greatly that you can't take Maliketh's side against Ranni, despite the fact that she basically caused much of what happened to happen in the first place. You literally just get, like... six or seven endings, with four of them being the exact same, but with different coats of paint. Bloodborne's endings in comparison were so wildly different from each other that they all felt incredibly unique, while Elden Ring really just got the shit end of the stick for one reason or another.

Also, Leda's message to the player implies a distinction between Empyreans and normal people with regard to the gate, "The Gate of Divinity lies in the tower sealed by shadow.
