You go in at day on a high difficulty, and they'll quickly spot you standing up so you need to crouch, stay low, avoid sightlines and use something like the drone and binoculars judiciously. Brush and plant life provide camo to navigate with, etc. Going in at night has always-accessible NVGs, enemy detection range is reduced, you can shoot out lights to keep the advantage. Beyond that, it's Ubisoft stealth detection: they have the awareness circle appear, sound cues, and if you get away from it they don't really have much depth beyond that and investigating, or going on caution at seeing dead bodies. You can move the bodies in Breakpoint, can't in Wildlands last I remember.
Like MGSV, there really isn't a solid indicator of how hidden you really are, unlike MGSV there isn't really a gear camo system since your clothing is entirely cosmetic. The stealth is important and functions, but you're not really getting guard sweeps in typical factors, more like enemies going around to check on suspicious spots, and in Breakpoint drones trying to one-up you to call in a cavalry or shoot your ass.
