They're actually pretty lenient with the digital stores these days unless you have an anime artstyle. Outlast Trials is WAY more graphic than Ready or Not and never had issues.
The difference is that VOID wants to do a physical release for Ready or Not. Outlast Trials had no physical release for this very reason, because they'd have had to censor it for it to be allowed on store shelves.

Apparently it also is the case that they lied to the ESRB. Someone went on ESRB's website and noticed that they found no indications of dismemberment whatsoever. So it's possible that VOID lied to the ESRB by only showing them the lack of dismemberment AFTER NPCs are killed (which the update disables dismemberment of).
