I don't get these. Why are they called this? They're typically just FPS games with a light RPG gimmick where you can heal by drinking booze and leave on the bathroom taps.
Duke Nukem had this though, so why isn't that an immersive sim? Is it because it lacked the minor RPG element?

Because Warren Spector came up with the term while developing Ultima VI and seeing a play tester mind control a rat to avoid an entire puzzle, something none of the developers expected

Because in Dukenukem combat is the default and only state of the game world. Imm-sims are more "life simulators", where things are calm and normal, enabling peaceful interactions with NPCs and the environment, until the player does something to disrupt the situation, IF they even choose to do so.
